It's a common North American phrase meaning that the girl in question is so sexually unappealing/STD ridden, that not only would I not want to touch her with my own sexual unit, but I would not risk yours either. While this phrase does suggest some camaraderie, as to say I wouldn't put you in the line of fire either, it also has some slight undertones that suggest that you are inferior.

e.g.
1. Yo man Shaniqua lookin pretty hawt today ya herd?
2. Goddamn white boy she got herpes! I wouldn't touch her with yo dick!
